The Accident Report form  
The accident report form, is to be used by companies to report when an accident or incident has occured.  
  (1) Where any accident occurs in a factory which either
    (a) causes loss of life to a person employed in the factory; or
    (b) disables any such person for more than two days from earning full wages at the work at which he was employed,
    the Manager of the factory, or person having control of the machinery in such factory shall forthwith report the occurrence of such accident to the Chief Factory Inspector and in connection therewith he shall furnish such particulars as the Chief Factory Inspector in any case from time to time require.
 
 
 
(2) The Manager of the factory or person having control of the machinery as aforesaid shall also from
  time to time in like manner report to the Chief Factory Inspector –
  (a) all accidental fires and explosions;
  (b) the collapse or failure of any building or structure;
  (c) accidents to machinery or plant which result in the cessation of work beyond the shift or day on which the accident occurs;
  (d) any industrial disease which may be prescribed by the Chief Factory Inspector, which may occur in the factory.

NOTE:- The provisions of subsections (1) and (2) quoted above also apply to:
(a) building operations undertaken by way of trade or business, or for the purpose of any industrial or commercial undertaking, and any line or siding which is used in connection therewith and for the purposes thereof.

It is a breach of The Factories Act to not report an accident, for which substantial penalties apply.

The Application for Registration or Re-registration form & Scale of Fees  
 

The application form is used to apply for registration of a factory. Under the Factories Act (1943) all establishments deemed as a factory are required to be registered under the Act. The registration period lasts three years and each owner/ manager of a factory is required to apply for a renewal of registration before the ending of the current registration period. A reminder will be sent when it becomes due.

It is a breach of The Factories Act to operate an unregistered factory, for which substantial penalties apply.

The registration process:

  1. The factory owner/ manager or person having control of the factory obtains and completes an application form. The form may be picked up at our offices or downloaded by clicking here. Ensure that the "name of factory" is the legally registered name of the factory (as registered with the Office of Registrar of Companies) and please be sure that the following sections are correctly completed: "the name of the owner (please view legal name)", "the nature of power used", "type of fire warning device" and "descriptive list of machinery".
  2. Based on the number of workers in the factory, the required fee (please see scale of fees) is to be paid at the most convenient tax office or collectorate. (Fees will not be accepted at any of our offices, or by any of our officers).
  3. The receipt is to be attached to the completed form and both sent in to one of our offices, most convenient to you.
  4. An inspector will then visit the establishment (usually takes approximately 1 month) if it has not been recently visited in our routine inspections, and make an assessment of the OSH conditions there. If satisfactory, a certificate of registration will be issued.

The First Aid requirements  
 

The Factories Act specifies in the "Second Schedule ", what it requires to be in each first aid box or cupboard. Factory owner/ managers are required to ensure that every first aid box or cupboard is placed under the charge of a responsible person who shall be available to render first-aid treatment whenever work is being caried on in the factory. In addition they are required to ensure that an adequate stock is available at all times, in good working condition.

The Report on Examination and Testing of Lifting Machines form  
 

This form (form C) is to be used by "competent examiners" only. The ministry certifies engineers operating private businesses to conduct statutory inspections on its behalf. These examiners appear on the "list of competent persons".
